LHENA Community Health Survey

LHENA is collaborating with HUE-MAN Partnership and the University of Minnesota School of Nursing in conducting a community-wide survey to self-identify health strengths, challenges, and needs of the Wedge neighborhood. LHENA will receive survey data to help facilitate positive change in our neighborhood to make it better and healthier for all. Survey results will also help the organization set a foundation for its strategic planning process for 2022 and beyond. 

If you are a resident of the Wedge neighborhood, you can take the community survey. It will take roughly 20 minutes to complete. A $10 gift card will be given to each participant when they complete the survey. A code generated at the end of the study should be sent to Clarence Jones at [email protected], and a gift card will be sent.

Survey results are confidential and only identifiable by zip code. The community survey findings will be presented to our neighborhood by the University of Minnesota in January 2022.
